Midazolam

Versed
Class:
Short-acting benzodiazepine CNS depressant.
Mechanism of Action:
  • Increased production of GABA, an inhibitory neurotransmitter, causing procedural amnesia; anesthesia; skeletal muscle relaxation.
  • Anxiolytic and sedative / hypnotic properties similar to other benzodiazepines.
Indications:
  • Status Seizure.
  • Sedation
  • Anxiolysis prior to oral or nasal intubation
  • Synchronized cardioversion.
Contraindications:
  • Glaucoma.
  • Shock.
  • Coma.
  • Alcohol intoxication.
  • Overdose patient.
  • Depressed vital signs.
  • Concomitant use with other CNS depressants, barbituates, alcohol, narcotics.
Adverse reactions:
Hypotension, respiratory depression and arrest.
Hiccouch, cough, over-sedation, nausea, vomiting, injection site pain, headache, blurred vision.
Adult Dosage and Administration
  • Seizures: 2-6 mg slow IV/IO/IM (may be repeated to total maximum: 0.1 mg/kg), 10mg IN
  • Cardioversion / Sedation: 0.5 - 2 mg IV/IO/IM/IN max 0.1 mg/kg
  • Intubation / Difficult Airway: 0.05 mg/kg IV/IO max 5mg. Repeat in 5-10 at half original dose x 2. Max 10 mg

Pediatric Dosage and Administration:
  • Seizures: 0.1 mg/kg IV/IO/IM Max 8mg or 0.2 mg/kg IN Max 10 mg
  • Cardioversion / Sedation: 0.05 - 0.1 mg/kg IV
  • Intubation / Difficult Airway: To facilitate intubation: Medical control may order:
    • 6 months - 5 years: 0.05-0.1 mg/kg IV maximum dose of 5 mg.
    • 6 - 12 years old: 0.1 mg/kg IV maximum dose of 8 mg.
Special Considerations:
  • Pregnancy safety: Category D.
  • Administer immediately prior to intubation procedure.
  • Requires continuous monitoring of respiratory and cardiac function.
